The service level is unticked for the prompt
When viewing a prompt set, the routines set against it will be displayed beside the prompts, with tickboxes for each service level. For each prompt, the appearance of the prompt on the task is determined by whether or not the service level is ticked.
The asset type hasn't been set for that prompt set
Prompt sets do not appear for any asset types unless specifically configured to do so. You can set which asset type the prompt set appears for by going to Control Panel > Prompt Sets > View > Update > Select applicable asset types in the Asset types field.
The routine service type hasn't been set for that prompt set

The standard of the prompt set doesn't match the standard of the routine service type
Prompt sets do not appear if the standard an the prompt set doesn't match the standard of the routine service type
You can check what standard the routine service type belongs to by going to Control Panel > Routines > Open the relevant Routine Service Type > Update
