How to add a take 5 form, create or edit dynamic forms and embed images

Setup

You will need the Dynamic forms module turned on in your workspace. If you do not see this in your main menu, reach out to [email protected].

You will also need the following permissions in your security group: Can change site configuration settings.

How to add a default form (take 5)

  1. Navigate to Control panel > Forms (v1) and click + Choose from library.

  2. Tick Require a Take 5 to be performed on all tasks and click Load.

  3. An action template required for all tasks with the take 5 dynamic form is automatically created.


How to create or edit a dynamic form

Navigate to Control panel > Forms (v1) and click + Create new form template or Edit existing on.

Editing templates requires advanced coding knowledge, and incorrect changes can lead to errors or inconsistencies. Changes to dynamic forms can be performed by the Uptick Professional Services team for an additional fee. To request a quote, please reach out to [email protected] with the following:

  • The name and the link to the template in the workspace.

  • Outline of the requirements for the customisation.

  • A PDF mockup highlighting the requirements.

How to embed images in the dynamic forms

1. Navigate to Control Panel > Document templates and scroll down to the Template Images section.

2. Upload images in the Template Images section, then edit an existing dynamic form to include it.
